Community Health Needs Assessment Survey

Dear Community Member,

Bon Secours Hampton Roads, Children’s Hospital of The King’s Daughters, Riverside Health System, Sentara Healthcare, the Hampton and Peninsula Health Districts and the Three Rivers Health District of the Virginia Department of Health are partnering to conduct a survey of the health needs in the communities we serve. These include the Eastern Shore, Middle Peninsula, Peninsula, South Hampton Roads, Western Tidewater, and North Eastern North Carolina. We would appreciate your participation in this assessment through the completion of a short survey. 

The past few years have highlighted many of the strengths of our communities, but they have also shown us that health disparities exist. We have this opportunity to prioritize areas together that will make our communities stronger. The 10-minute survey will assist us in identifying the most pressing health concerns and service gaps in our area so we can prioritize our efforts. Your ideas and suggestions will help us work with key stakeholders, both internally and externally, to develop strategies that meet individuals where they live, work, learn and play.

The survey link below is administered through the online tool Survey Monkey.  Your responses will be analyzed as part of our community health needs assessment.  The assessment results will be shared with the community through each health systems’ websites. We look forward to developing actionable plans that address the identified needs outlined in the assessment.
